2. In the event of not being able to make direct contact with parents, I grant permission for my child to receive medical treatment in the instance of an emergency.

3. By signing this form I am agreeing to enroll my child into Aligarh International School (AIS) and give my consent that he/she is allowed on supervised field visits under appropriate school guidance and controls.

4. I declare that all the above information is authentic and true to the best of my knowledge, found untrue I accept the admission may stand cancelled.

5. I further undertake that I have submitted duly attested xerox copy of the following documents: I). Birth Certificate, II). Marks Sheet of last examination passed, III). Stamp Size recent Coloured Photograph of the child, IV). Coloured Photograph of the child with parents.
